The Nutrition and Dietetics team at Fairfield Hospital includes clinical dietitians, diet supervisors and dietitian assistants. We service Fairfield Hospital's inpatient wards and provide outpatient services. 

Who we care for

We care for and provide nutrition and dietary assessment, education and counselling to people living within the Fairfield Local Government Area for various clinical conditions such as: Malnutrition, Renal conditions, Diabetes, Cardiovascular Disease, Paediatric conditions.

How to access our service and eligibility

If you are an inpatient and would like to speak with a dietitian, please tell your doctor, midwife or nurse. They can arrange for a dietitian to see you. 

If you are an outpatient, your GP or specialist are always your first step in seeking treatment for any clinical condition. You can be referred to our service if your treating doctor feels your treatment requires Dietitian input and you are eligible. Once we receive the referral, it will be reviewed by our service, and you will be contacted to book the appointment.

Please note that calls from the hospital will appear as a blocked number. Please answer our call and we request you to turn on your voicemail so we can leave a message.

What to bring to your appointment

  • Your Medicare/DVA card
  • Your referral letter from your GP/Specialist 
  • List of current medications and drug allergies 
  • Recent and relevant blood test results

Allied Health Services at Fairfield Hospital

Allied Health professionals are an essential part of the patient care team at Fairfield Hospital. The allied health team comprises Occupational Therapists, Allied Health Assistants, Physiotherapists, Social Workers, Aboriginal Liaison Officers,  Dietitians and Diet Aids, Speech Pathologists, and Exercise Physiologists to support patients throughout their recovery and ongoing health needs.

Across inpatient and outpatient settings, our Allied Health teams:

  • Help patients regain function, mobility, independence, and confidence
  • Provide assessment, therapy, and education tailored to individual needs
  • Support communication, nutritional wellbeing, and psychosocial health
  • Work closely with medical and nursing teams to deliver coordinated, patient-centred care
  • Assist with discharge planning and smooth transitions back to home or community services

Our goal is to improve health outcomes and quality of life for every patient by delivering safe, evidence-based care in a compassionate and collaborative environment.

 

Digital Referral Pathways to SWSLHD

South Western Sydney Local Health District receives outpatient referrals through Consultmed, our secure online referral platform. This helps referrals reach the right service and supports timely review and processing.


Referrals can be submitted in one of three ways

1. For your GP or other Healthcare Provider

Please speak with your GP, specialist, allied health professional or primary care provider and ask them to submit your referral through Consultmed.

2. For NSW Health Staff

NSW Health staff can submit referrals to SWSLHD services through Consultmed.

If you already have a referral letter, you can upload it directly through the SWSLHD self-referral portal.

Important Information

Please make sure your referral includes:

  • Your personal details
  • The reason for referral
  • Relevant clinical information
  • Supporting documents such as imaging or test results

Once submitted, your referral will be reviewed by the relevant hospital team. You may be contacted if additional information is required.
